Welcome to Franklin Township Public Schools,

Please use the link below to register as the process is all online. Please ensure the below are completed for your child and uploaded as well during the registration process.  It is recommended that you have all documents, including any academic records, as well as, if applicable, IEP or 504 plans ready for upload either on your pc or phone prior to beginning the process as the system does time out after a period of inactivity. Also, please ensure all fields are completed. If the question doesn't apply please indicate NO or N/A. If you don't receive an email with a confirmation number, the registration is not complete and you should go back and check for any errors. 

Important Registration Documents - These Will Have To Be Uploaded In The Document Section During The Online Registration Process 

• Primary Proof of Residency or Residency Information Sheet (if applicable)- Only Required If Lease or Deed Is Not In Either Parent/ Legal Guardians Name

• Health History Form - Must be completed by parent/legal guardian

• Dental Form - Must be completed by a US Dentist - Mandatory 

• Universal Child Health Record Form - Must be completed by a US Physician - Mandatory

• SEMI Form - Must be completed by parent/legal guardian

• Records Release Form - Must be completed by parent/legal guardian

• Immunizations - Must be current from the physician

• Parent Photo ID - Both parents/guardians - (TO CHANGE YOUR ADDRESS WITH NJ DMV ONLINE)

• Birth Certificate for child begin registered- Legible Copy 

• Custody Documentation (if applicable) - Must be legal documents from court/attorney
